Guidance on Completion of a Disclosure and Barring Service (DBS) Application

All doctors, dentists and opticians must undergo an Enhanced DBS check when they apply for inclusion in a Performers List in Wales. There is no charge to the applicant for this service.

Applicants that subscribe to the DBS update service will be required to provide their latest original DBS certificate plus supporting identity documents (as detailed below).  You will be asked to provide written consent to enable a DBS status check to be undertaken when you attend for your face to face interview.

Completing the DBS (formerly the Criminal Record Bureau) Disclosure Application Form

Please note the following important points when completing your DBS Disclosure application form:

 

  • Photocopies of the application form or supporting documentation are not acceptable. Only original forms and documents may be provided.
  • From the List of Valid Identity Documents (Appendix A) you must supply 3 documents in total; 1 document from Group 1 and 2 further documents from Group 1, 2a or 2b; one of which must verify your current address. If you are unable to produce these documents please contact the NHS Wales Shared Service Partnership for advice (contact details given below).
  • Use black ink when completing this form and insert only one character in each box.
  • Use capital letters when completing this form.
  • All sections in yellow and their associated fields are mandatory and must be completed.
  • Do not complete section D – this is currently not required.
  • If a field does not apply to you, please leave it blank. Do not enter N/A or any variation.
  • If you make a mistake, put a line through and correct it to the right. Do not use correction fluid.
  • Ensure you supply all of the addresses you have lived at in the last five years including any overseas addresses.  Your form will be rejected by the DBS if there are any gaps in the information provided.
  • Ensure you supply all names by which you have been known.
  • If there is insufficient space on the application form, download and complete a continuation sheet from www.homeoffice.gov.uk/dbs-continuation-sheet. If you do not have access to the internet, you can request this from the NHS Wales Shared Service Partnership (contact details given below).
  • If you have selected more than one of the “YES/NO” options in error, place a cross in the correct box and circle it.
  • Keep your signatures within the provided box.
  • You must not include stamps or stickers on the form.
  • Transgender applicants should contact the DBS Transgender Line on 0151 676 1452 or email sensitive@dbs.gsi.gov.uk for further advice about completing the form.
