Incident Investigation

SMTL have extensive experience of undertaking medical device and pharmaceutical adverse incident investigations for the NHS and commercial partners. We have several staff that have acted as expert witnesses in legal disputes and have given evidence in coroner courts where medical devices have been implicated in patient death.

Our investigations are undertaken by trained and qualified personnel. It is imperative that any samples taken, analysed or tested are traceable so that a chain of custody can be established. All work is carried out diligently in accordance with our quality systems to ensure confidence in the results of any investigation.

Examples of recent projects include examination of a potential wound dressing discharged from a patient, investigation of surgical instrument corrosion and characterisation of particle matter contamination of pharmaceutical preparations.
