All Occupational Health referrals must be completed by the Single Lead Employer Team.
If you would like to request a referral for one of your Resident Doctors, Dentists or Trainee Pharmacist, please complete the form below.
Please ensure you provide as much information as possible in this form including:
Reason for referral - details of health condition / concerns
What issues / concerns would the employee like to discuss with the Occupational Health team
Preferrable outcome of referral, e.g. support on phased return, recommended adjustments
YOU MUST HAVE CONSENT FROM THE EMPLOYEE TO REQUEST A REFERRAL
