If you are a Returning GP and wish to assist NHS Wales in the provision of General Medical Services during the COVID-19 emergency, you must complete an application to join the Medical Performers List in Wales and hold a valid Enhanced DBS Certificate with both the Children and Adult Barred Lists checked.

We strongly recommend that you read the guidance notes before submitting an application.  The guidance in the link below will give you instructions on completing and submitting the MPL application form and what documents you need to provide for your DBS clearance.

If you hold an Enhanced DBS Certificate with both the Children and Adult Barred Lists checked which is dated within the last 3 years, please submit a scanned image of this Certificate with your application.

If you hold an Enhanced DBS Certificate and are registered with the DBS Update Service, please submit a scanned image of your latest DBS certificate and a signed copy of the Update Service Consent Form available via the link below:

If you do not hold a valid Enhanced DBS Certificate with both the Children and Adult Barred Lists checked, or you are not part of the DBS Update Service, you can apply for a new DBS Certificate via the link below:
