Apply for Inclusion in the Dental Performers List

2. DBS

If you are applying for inclusion in a Dental Performers List in Wales one of the requirements of the application process is that you provide an Enhanced DBS disclosure certificate (which includes children and vulnerable adult barred list checks) that has been issued in the last six months, or an older certificate which is registered with the DBS Update Service. If you do not have an appropriate DBS certificate, then you can apply for a new one here: - Online Disclosures. You will need to read the guidance notes entitled “Completing the online Disclosure and Barring Service Application Form” as these notes contain the password and code needed to complete the DBS application form.

Once you have completed the online DBS application form, we will need to verify your identity before we can submit the application.

If you have a valid UK passport or a Home Office Digital Share Code, then you can verify your identity via Trust ID. Please email nwssp-primarycareservices@wales.nhs.uk requesting a Trust ID link confirming your name and the email address you wish to use. Please use “Dental Performers List” in the subject header. This is the quickest route to confirm your identity. Please note you will need to present your original identity documents at any practice you work. Please note the Trust ID link will not be sent to you until you have also submitted your application form to join the Performers List.

If you do not have a valid passport or Digital Share Code, then you will be required to submit your original ID documents to this office and undertake a video link appointment to confirm your identity. Please email nwssp-primarycareservices@wales.nhs.uk requesting a video appointment. Please use “Dental Performers List” in the subject header. Please note, a video appointment cannot take place until you have submitted your application form to join the Performers List and your original ID documents. 3. Application

You must complete:

• Form DPL 1 (if you wish to be included in a Dental Performers List); or
• Form DPL 5 (if you are already included in a Performers List in the UK)
• Form IMP1 (for dentists applying for Inclusion via Mentorship Programme)
• Form IMP2 (Mentor application)

We strongly recommend that you read the guidance notes before completing the application form.
