Requesting Access to Health Records
In order to access GP health records for an individual who is unregistered or deceased you will need to complete one of the below applications and submit this, along with copies of your identification documents, to the following address;

If you are making the application as an individual’s legal representative, you will need to provide signed and dated authorisation for you to act on their behalf.

Timescales and Processing
We have 40 calendar days from receipt of a fully completed application to provide you with copies of a deceased patients GP record. In some instances this may not be possible due to circumstances beyond our control (for example, where the record is currently in transit from the GP surgery). We will update the applicant on the progress of their request if this is the case.
We have 30 calendar days from receipt of a fully completed application to provide you with copies of an unregistered patients GP record.
Please note, we process applications for the records of patients who were previously registered in Wales only.
For hospital records, you will need to contact the relevant Hospital Trust.
