To receive a smartcard please complete the "Professional Registration Application" form. Once this form is completed you will then receive a notification to the email address provided in the application form inviting you to start your Apply for Care ID Application.
Once this stage is completed and your application has been approved, your smartcard will be printed and sent to your address stated in the application
Important Information for Applicants in Wales: Priority for EPS Rollout
Due to the high volume of applications in Wales as part of the Electronic Prescription Service (EPS) rollout, priority will be given to applications associated with this programme.

Existing Smart Card Holders
If you are already in possession of a smart card, you do not need to apply for a new one to work in Wales. Your existing smart card will remain valid and functional for use in Wales
