Applying for a Smartcard as a Locum

If you are a locum GP/Pharmacist and work in an EPS enabled site you will now be required to obtain a smartcard

To receive a smartcard please complete the "Locum Smartcard Notification" form. Once this form is completed you will then receive an email to the email address provided in the application form inviting you to start your Apply for Care ID Application.

As soon as this is completed and been approved a request will be made to get your smartcard printed and delivered to the delivery address supplied on the application form.

Important Information for Applicants in Wales: Priority for EPS Rollout

Due to the high volume of applications in Wales as part of the Electronic Prescribing Service (EPS) rollout, priority will be given to applications associated with this programme.

Application Process

  1. Complete the Form
    Once you have submitted the completed form, you will receive a notification to begin the care application process.
  2. ID Verification and Approval
    Your application will undergo the necessary identity checks. Upon approval, your smart card will be dispatched to you by post.

Interim Access for EPS-Enabled Sites

If you are working at an EPS-enabled site and awaiting your smart card, please coordinate with your site to use a Temporary Access Card (TAC) during this period.

Existing Smart Card Holders

If you are already in possession of a smart card, you do not need to apply for a new one to work in Wales. Your existing smart card will remain valid and functional for use in Wales.
