Step by Step Guide to Submitting Your Account

• Sort your account, remember to separate forms into groups 1, 1A and 2
• Clearly identify Group 1A prescriptions within the bundle by use of the separator slip
• Please DO NOT attach adhesive tape, labels, pins or staples to prescriptions.
• Invoices are not required providing all information has been endorsed i.e. Maker or supplier, net cost and pack size
• Complete all parts of the WP34D. REMEMBER you can submit your Multi Vaccine claims on line using EMVC.

NB If you submit Multi Vaccines via EMVC we do not require a paper copy of the Appendix or a paper copy of your on line submissions.

• Address labels can be downloaded
• Postage on parcels and correspondence MUST be pre-paid
• A courier service is available
• Submit by the submission deadline.

Please Note:

You do not need to separate bar coded forms from non barcoded in any of the groups.
